What is clinic for:
Outpatient Department for aged care and rehabilitation
Who can be referred to outpatient clinic:
- Patients over the age of 65
- Patients who live in Northern Suburbs
- DVA, public and private patients
- Patients without a valid Medicare card can be referred to Osborne Park Hospital
Services provided:
- Dietetics, Physiotherapy, Occupational therapy, Speech therapy, nursing, social work services
- Home visits for occupational therapy
- Medical Clinics which include, Osteoporosis Clinic, Stroke disease Clinic, Parkinson’s disease Clinic, Fall’s clinic, Memory Evaluation Clinic and General Medical run clinic.
- One way taxi service to appointment can also be provided
How to refer to clinic:
- Refer directly to the Day Therapy Unit
- Refer patient to Aged Care Assessment Team (ACAT) at Osborne Park Hospital
Costs of clinic appointment:
- Public clinic
- There may be a small fee for medical consumables
What to bring to appointment:
- Medical clinic appointment – medication profile, referral and any recent x-rays
- Medicare card and health fund details
