Palliative Care

The Palliative Care team at Joondalup health campus provides comprehensive care to patients with a life limiting illness and their families. We have a dedicated multidisciplinary team consisting of Specialist Palliative Care Doctors, a Palliative Care Clinical Nurse Specialist and Palliative Care Clinical Nurses, Social workers, Occupational Therapists, Physiotherapists and Pastoral Care workers.

We provide a consultation service throughout the hospital (for both private and public patients) and have a 10 bed public inpatient Palliative Care Unit, located on ward A1b, which can be accessed through the medical suites above Pause café and the hospital pharmacy.

Services Provided:

Our team can assist with symptom management, advanced care planning and end-of-life care.

We have strong links to external Palliative Care teams in other hospitals, community based Palliative Care services, Acute Pain Services, Oncology, Psychiatry and various other networks involved in Palliative Care around the state of Western Australia.

Referral process:-

Referrals to the Palliative Care Unit can only be made by other hospitals/Community Specialist Palliative Care Services (we are not currently excepting referrals from GP’s)

To make a referral:

Phone (08) 9400 9573 and provide a verbal referral to the Palliative Care Team.

We will then triage and waitlist the patient. When a bed becomes available, we will phone the referrer and/or the patient and advise them to come in that day, prior to 12pm.

If urgent assistance is required, please go to the Emergency department and our team can provide input, via consultation.

Hours of operation:

Our Palliative Care team operates Monday – Friday 0800 – 1600.

A Specialist Doctor is also available on-call 24/7, to provide phone advice to doctors and nurses, regarding patients who are currently located within the hospital.
