For Patients: What to bring

What to bring

• A list of current medications you are taking either from your Pharmacist or Doctor
• Any letter(s) from your doctor.
• X-ray films, discs and/or reports.
• All medications you are currently taking.
• Your blood group card (if you have one).
• Your Medicare card.
• Any Pension or Concession cards.
• Your Department of Veterans' Affairs entitlement card (if applicable).
• Your Health Fund details.
• Your Safety Net card.
• Night attire, dressing gown and slippers (non-slip soles).
• Personal toiletries, including tissues.

What not to bring

• Do not bring valuables such as large sums of money or jewellery, as the hospital will not accept responsibility should they become lost or damaged.
